Come and talk with other youth and staff from the American Family Children’s Hospital, the Waisman Center, Kids As Self Advocates, Family Voices of Wisconsin, and the University of Wisconsin Pediatric Pulmonary Center to learn more about transition to adult health care.Saturday, November 8, 2008 10:00 am - Noon American Family Children’s Hospital Madison, Wisconsin Come and talk with other youth and staff from the American Family Children’s Hospital, the Waisman Center, Kids As Self Advocates, Family Voices of Wisconsin, and the University of Wisconsin Pediatric Pulmonary Center to learn more about transition to adult health care. You will have an opportunity to think more about how you want to be involved in your health care. Topics will include:

  • What you need to do when you are sick
  • How to set up an appointment and talk with your doctor
  • Know and order your own medications
  • Decision-making at age 18

Teens are welcome to invite a friend or sibling to attend the youth session with them. *A separate presentation is scheduled at the same time just for parents and caregivers.
